Blood Bowl is a fantasy football video game that was adapted from the board game of the same name. It is played between two teams of up to 16 players. Each team can have up to 11 players on the field at any time. A team can win by scoring touchdown or by completely eliminating the entire roster of the opposite team. The game has both real time and turn-based strategy modes.

During career mode, players must create their own team. As the team ranks up the tournament ladder a player will be able to sign on better players on the roster. If a character gets killed in the game, the player will need to replace them for one to several matches until the character is able to return. If the player lacks the money to heal the character, the player can still use an injured player but the chance of them getting killed increases.

The characters on the roster can take part in tournaments where they can gain experience and level up. Players will be able to assign skill points to buy new abilites that will improve dice rolls, give them a second chance on a bad roll and other bonuses. Leagues are completely customizeable. There are 25 elements that can be changed.

The DS and PSP versions do not have career mode available and do not offer customization options. Multiplayer involves turn-based hotseat gameplay on the handheld versions, though the PSP version also includes wireless multiplayer
